I think the 2007 Case Book provides some insight on page 25 concerning substitutions. The foul occurs simultaneously with the snap. If the QB is checking the defense an sees an extra player, he should snap the ball immediately to draw the penalty. I agree that in either the case of A or B, if the replaced player does not attempt to leave the field immediately it should be flag at that point.
I agree. Put once the center places his hands on the football, isn't a snap about to happen?
